By accepting & Submitting this Angry Heat Level Waiver and Release of Liability Form I hereby release ANGRY CHICKZ INC. from any liability, whatsoever with regards to The Angry Heat Level.

I acknowledge that there are certain risks associated with the activity of consuming the Angry Heat Level. The risk and dangers are but not limited to increased use of profanity, burning sensation of the mouth, lips, profuse sweating, bodily injury or even death.
